There were a handful of derailments back in the days when I was relying on the L-Taraval to get to work and home again, but I don't recall that any of them involved injuries. There were many stalls circa 1979-81 when the passengers had to disembark west of the closed Eureka Street station and walk through the tunnel to exit at Castro & Market.
