Hi... I did a little poking around online. Apparently, it was a self-published book. So it would not be available in regular bookstores, or on sites selling used books.
There was a copy on eBay - but is now no longer available. This was not a bound book, but a spiral-bound "book." You can see a photo of it here: https://www.ebay.com/itm/195755474125
The SF Public Library - Main Branch has a copy, but it's for in-house reference use only, and available in the Stacks for viewing, but not checking out.
